Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Please arrive a few minutes early for this appointment to allow the completion of necessary information reguarding your health and other vital statistics. Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
* Your surgical referral form from your primary doctor and any X-rays that are appropriate.
* A list of any medications you may be taking.
* If you have dental insurance, bring evidence of current coverage, a card with group and policy numbers, and necessary employer information.
*Please alert the office if you have any medical condition that may be of concern prior to surgery, (i.e. high blood pressure, diabetes, artificial heart valve, asthma, blood thinner, orthopedic joint prosthesis, recent stroke or heart attack…)
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
